USE OF FACILITY WAIVER, RELEASE OF LIABILITY & PRIVATE ASSOCIATION PRACTICES
TRINITY GLOBAL MINISTRIES/WE RISE! FREEDOM LEARNING CENTER AND ALL AFFILIATES
THIS IS A RELEASE OF LIABILITY. YOU AGREE THAT IT IS COMPLETELY YOUR RESPONSIBILITY IF YOU OR ANYONE ATTENDING WITH YOU ARE INJURED IN ANY WAY. YOU MUST CAREFULLY READ AND FULLY UNDERSTAND THIS BEFORE SIGNING.
I have decided to voluntarily participate in COURSES/EVENTS/CLASSES/FOOD/CONSUMABLES and to use the FACILITY OR BE PHYSICALLY ON THE PREMISES AT WE RISE! FREEDOM LEARNING CENTER/TRINITY GLOBAL MINISTRIES. I understand and agree that physical, property damage injuries can occur; accidents happen; including food allergies or sickness. Although, activities conducted on the property are not normally dangerous, accidents and sickness can still occur. Whether through playing games, walking around, running, consuming food and drink etc. (all of these harms, including those not listed or unknown, are collectively referred to as “Harm” in the rest of this document).
Some risks that may cause Harm include: falling; coming into contact with other people, visible or hidden defects, or conditions on the property; and through intentional or unintentional acts and negligent acts by myself or other people. I understand and agree that this list of risks is not complete and that other known or unknown risks may also cause Harm to myself, property, or other people.
I voluntarily agree and promise, on behalf of myself, my family, and/or anyone in my care, to accept and assume all responsibility, financial or otherwise, for any injury, death, illness, damage, or any other Harm that happens to me or to my property because I chose to participate in any program at the Center. Including simply being present on the premises. I understand that by signing, this is legally binding. I understand and agree that by signing this document, I am giving up my legal rights to seek any compensation or reimbursement for any Harm to myself, my property, and any Harm to any children in my care no matter how the Harm was caused.
I understand that I agree to, on behalf of any Minor in my care, the same exact release of legal rights included in the complete Waiver and Release of Liability. On any Minor’s behalf, I consent to and ratify their agreement to the Release included in the Waiver and to the entire Waiver.
I also understand and agree that Trinity Global Ministries/We Rise! Freedom Learning Center assumes no liability for any loss, damage, or any kind of injury, harm or illness to myself or my property while I or any child under my care use the Center and the surrounding facility and areas operated by or hosted by the Center (those places, the “Premises”), and the equipment provided by the Center (the “Equipment”). The Equipment includes but is not limited to games, outside property, materials used in and outside of class, etc.
By signing this waiver and release of liability and using the premises, and any equipment, I agree to fully and forever release from responsibility and agree to indemnify and hold harmless the following entities: Trinity Global Ministries, We Rise! Freedom Learning Center, any owners, direct and indirect parents, subsidiaries and other affiliated entities (including the foregoing’s direct and indirect parents, subsidiaries and other affiliated entities), all of their respective successors, assigns, and current or future officers, directors, managers, members, partners, employees from any and all Harm, liabilities, claims, demands, damages, rights of action, suits or causes of action, present or future, whether the same be known or unknown, anticipated or unanticipated, resulting from or arising out of my use or intended use of We Rise! Freedom Learning Center/Trinity Global Ministries, the Premises and the Equipment, or otherwise arising out of my presence or activities at the Center and on premises.
I am aware and agree that this waiver's validity does not have an end date and does not expire. Validated on date of signature.
